1. Statutory Framework

2. Procedure

2.1 Purpose

The purpose of the Family Support Summary Sheet is to provide the child's social worker with a succinct and meaningful summary of family support intervention. The summary sheet can be used:

  • By the child's social worker in informing current and future assessments and plans for the family;
  • In the preparation of court statements and therefore evidenced based reporting is necessary;
  • For reporting to child protection conference.

2.2 Reviews

Reviews for family support should be completed following 12 weeks of intervention. However, there may be occasions when periods of intervention are shorter than this; for example, where a family disengage.

2.3 Continued support

Where a new role or continued work for family support is identified at the 12 week review, a new event and a new family support summary will be required.
